Practice Closing

To My Patients,

On Friday, September 17, 2021 at the end of the business day, I will be permanently closing my Medical Practice, to begin my new role as Associate Medical Director, Pain Management and Physiatry, for a national healthcare organization.

Please contact your primary care physician or the provider who referred you to my office to receive ongoing and/or future treatment. If you don’t have a primary care physician or referring provider, you may contact the Onondaga County Medical Society (315-424-8118) or the local medical society in your own county to find another medical provider.

After September 17, 2021, I will be unable to renew any medication prescriptions, physical therapy orders, complete any disability forms, order any diagnostic studies, or process any other type of paperwork, without exception. Your Patient Portal will be deactivated, you will be unable to leave voicemail messages or send emails or faxes to our office, and any mailed documents will be returned to sender. Please contact your pharmacy, physical therapist, insurance carrier, attorney, or others upon receipt of this letter to ensure that such requests and documents are forwarded to another provider.

Shortly after September 17, 2021, your electronic health records will be transferred to our records custodian, Morgan Records Management, LLC, in an encrypted electronic format, who will continue to maintain your records in accordance with applicable confidentiality and security standards, for no less than six (6) years after the last date of service you received from this Medical Practice, after which your records be securely destroyed, in accordance with NYS Department of Health Regulations.

If you wish to obtain a copy of your medical records, you must contact the Records Custodian using one of the methods noted below, and your signature will be required on a records release form. You will be required to pay a fee to obtain your medical records, depending upon how you choose to receive them.

It has been a great privilege to provide medical care to the people of Central New York over the last 32 years, and while I will miss our time together, I look forward to my new career as an opportunity to continue to help patients and their providers on a nationwide basis.
